This can be reproduced by opening Visual Studio, going to File > New > File > D > Empty D Module, and editing the file. It happens sporadically; I have been unable to reproduce it since yesterday, so it may have been a problem with my system, as I have rebooted since then.

I could reproduce it by triggering a completion on the very first character of the source file. I have uploaded a new beta here: https://github.com/D-Programming-Language/visuald/releases/tag/v0.3.40-beta1

I have not encountered this issue since installing the new beta, so I'm going to close this.
