E.g.:
htod cairo-win32.h
generates cairo-win32.d
This won't compile.
I think a safe option would be to convert to underscores:
cairo_win32.d
and also do the same for the module declaration in the file.

Ah but I can already do this:
htod cairo-win32.h cairo_win32.d
I guess I should close it then.
